Below is a circular published by the state of Florida in 1978. I think it is no longer in print though I have a hard copy. It is reproduced below. Visual quality varied on the original typewritten copy. There is no intent to violate copyright but just to provide information written before the Internet to the Internet. I have moved the index to the front rather than leaving it at the end. The index is by number not page.
